The levels of glycohemoglobin in the blood and glucose, lactate, and acetoacetate in the vitreous body of the eyes were determined. The diagnosis of diabetic coma was made taking into account previously developed criteria.</p> <p><bold><italic>RESULTS</italic></bold><italic>: </italic>A total of 692 (15.6%) patients with diabetes mellitus died of diabetic coma. Diabetic coma-related deaths occupy a much larger percentage of cases than is available in statistical data because of the insufficient implementation of laboratory tests at the post-mortem stage of diagnosis and restricted coding of the cause of diabetic coma-related death of patients with diabetes mellitus. Methods for diagnosing diabetic comas are easy to perform, guarantee high accuracy and reliability of results, and are available for any specialized laboratory.</p> <p><bold><italic>CONCLUSION</italic></bold><italic>: </italic>The most common precomatous state (the beginning of the development of hyperosmolar non-ketoacidotic coma), which in combination with coma itself, leads to the highest rates of diabetic coma-related death. In recent years, diabetic coma-related death tended to reduce.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование</italic></bold><italic>. </italic>Данные статистики о причинах смерти больных сахарным диабетом не соответствуют реальным показателям в связи с недостаточным уровнем диагностики на постмортальном этапе исследований в большинстве регионов страны.</p> <p><bold><italic>Цель исследования</italic></bold> ― статистический анализ смертности больных сахарным диабетом в результате диабетических ком за 2017–2020 гг.</p> <p><bold><italic>Материалы и методы</italic></bold><italic>. </italic>Проведён биохимический анализ крови и стекловидного тела глаза. В крови определяли содержание гликогемоглобина для подтверждения наличия сахарного диабета. В стекловидном теле глаза определяли содержание глюкозы, лактата и ацетоацетата. Критерии дифференциальной диагностики диабетических ком по биохимическому анализу стекловидного тела глаза были разработаны нами ранее.</p> <p><bold><italic>Результаты</italic></bold><italic>. </italic>Исследование проведено на трупном материале 4444 больных сахарным диабетом, скончавшихся скоропостижно (в катамнезе стоявших на учёте при жизни с диагнозом сахарного диабета). Все больные сахарным диабетом имели повышенное содержание гликогемоглобина. При анализе структуры смертности больных сахарным диабетом установлено, что в результате диабетических ком скончалось 692 человека, что составило 15,6%. В структуре смертности больных сахарным диабетом диабетические комы занимают значительно больший процент случаев, чем указывается в статистических данных. Данное положение обусловлено недостаточным внедрением лабораторных исследований на постмортальном этапе диагностики, а также ограничением кодирования причины смерти больных сахарным диабетом в результате диабетических ком. Разработанные способы диагностики диабетических ком просты в исполнении, гарантируют высокую точность и достоверность результата, доступны для любой специализированной лаборатории.</p> <p><bold><italic>Заключение</italic></bold><italic>. </italic>Наиболее часто встречается прекоматозное состояние (начало развития гиперосмолярной некетоацидотической комы), которое в сочетании с самой комой даёт наибольший процент смертности в результате диабетических ком. В последние годы отмечается тенденция к снижению смертности в результате диабетических ком.</p></trans-abstract><trans-abstract xml:lang="zh"><p>论证。由于国内大多数地区在尸检阶段中的诊断不足，关于糖尿病病人的死亡原因信息不符合现实。</p> <p>研究目的是在2017-2020年内由于糖尿病昏迷导致的糖尿病患者死亡率的统计分析。</p> <p>材料与方法。进行了血液检测和玻璃体的生物化学分析。测定血液中的糖化血红蛋白以确认是否存在糖尿病。测定玻璃体中的葡萄糖、乳酸盐和丁铜酸酯的含量。我们之前已制定了通过玻璃体的生化分析对糖尿病昏迷进行鉴别诊断的标准。</p> <p>结果。这项研究是在4444名猝然去世的糖尿病患者的尸体上进行的（这些患者在生前被登记为糖尿病病人）所有糖尿病患者的糖化血红蛋白水平都提高了。在分析糖尿病患者的死亡率结构时，发现了692个人的死因是糖尿病昏迷，占15.6%。在糖尿病患者的死亡率结构中，糖尿病昏迷的比例比统计数据中注明的的要高得多。这种情况是由于在死后诊断阶段中采用的实验室检查不足，以及糖尿病患者由于糖尿病昏迷导致的死因编码有限。制定的糖尿病昏迷诊断方式是易于执行的，确保结果的很高的准确性和可靠性，任何专业实验室都可以进行。</p> <p>结论。最常见的是昏迷前状态（高渗性非酮症酸中毒昏迷的发作），其与昏迷结合在一起，使糖尿病昏迷的死亡率百分比最高。近年来，糖尿病昏迷导致的死亡率有下降的趋势。</p></trans-abstract><kwd-group xml:lang="en"><kwd>diabetes mellitus</kwd><kwd>diabetic comas</kwd><kwd>mortality</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>сахарный диабет</kwd><kwd>диабетические комы</kwd><kwd>смертность</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>糖尿病</kwd><kwd>糖尿病昏迷</kwd><kwd>死亡率</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Dedov II, ShestakovaMV, Vikulova OK, et al.
