Standard analytical methods, designed for the detection of known compounds, are not oriented towards identifying new analogues, thereby increasing the risk of diagnostic errors. In this context, high-resolution mass spectrometry (HRMS) is regarded as a platform capable of providing the necessary level of evidentiary value due to its combination of high specificity, sensitivity, and the capability for retrospective data analysis. The implementation of HRMS into routine practice in the Russian Federation is limited by the lack of systematic data on its diagnostic accuracy. This review analyzed 53 sources selected in accordance with the PRISMA-2020 guidelines. The search was conducted in the PubMed, ScienceDirect, eLibrary, and Google Scholar databases. It was demonstrated that QTOF and Orbitrap analyzers enable NPS identification with a mass error of less than 5 ppm, allowing for the differentiation of isomers and the determination of empirical formulas for unknown compounds. Various screening modes are described, including targeted analysis, suspect screening using databases (HighResNPS), and non-targeted analysis. The effectiveness of the retrospective approach, which allows for the detection of NPS in previously acquired data without re-analyzing samples, was shown. Data on the method's sensitivity were systematized; limits of detection in blood and urine reach sub-ng/mL concentrations, which is sufficient for identifying NPS in both acute poisoning cases and at trace levels. The impact of matrix effects on analytical accuracy and strategies for their minimization, including the use of isotopically labeled standards, were examined. The specific aspects of working with various biological specimens (blood, urine, hair), considering NPS metabolism, as well as autopsy material, were addressed separately. Practical recommendations for the implementation of HRMS into the practice of domestic laboratories were formulated.</p></abstract><trans-abstract xml:lang="ru"><p>Стремительный рост распространенности новых психоактивных веществ и постоянное обновление ставят новые задачи для судебно-токсикологического анализа. Стандартные методы анализа, ориентированные на детекцию известных соединений, не нацелены на поиск новых аналогов, что повышает риск диагностических ошибок. В этих условиях масс-спектрометрия высокого разрешения рассматривается в качестве платформы, которая способна обеспечить необходимый уровень доказательности благодаря сочетанию высокой специфичности, чувствительности и возможности ретроспективного анализа данных. Внедрение ВР-МС в рутинную практику в РФ ограничено отсутствием систематизированных данных о её диагностической точности. В настоящем обзоре проведен анализ 53 источников, отобранных в соответствии с рекомендациями PRISMA-2020. Поиск проводился в базах данных PubMed, ScienceDirect, eLibrary и Google Scholar. Продемонстрировано, что анализаторы QTOF и Orbitrap обеспечивают идентификацию НПВ с погрешностью менее 5 ppm, позволяя дифференцировать изомеры и устанавливать брутто-формулы неизвестных соединений. Описаны такие режимы скрининга, как таргетный анализ, suspect-скрининг по базам данных (HighResNPS) и нетаргетный анализ. Показана эффективность ретроспективного подхода, позволяющего выявлять НПВ в ранее полученных данных без повторного исследования образцов. Систематизированы данные о чувствительности метода, пределы обнаружения в крови и моче, которые достигают суб-нг/мл концентраций, что достаточно для выявления НПВ как при острых отравлениях, так и следовых концентраций. Рассмотрено влияние матричных эффектов на точность анализа и стратегии их минимизации, включая применение изотопно-меченых стандартов. Отдельно рассмотрена специфика работы с различными биологическими образцами (кровь, моча, волосы) с учетом метаболизма НПВ, а также аутопсийных материалов.
