Currently available forensic medical data about this nosology were produced from samples obtained from corpses of individuals who has not undergone any previous therapeutic and diagnostic procedure. The dynamics and role of metabolic disorders in the thanatogenesis of acute methadone poisoning and the forensic assessment of such cases have not been widely studied in forensic medicine.</p> <p><bold>AIM</bold><italic>:</italic><italic> </italic>To perform a forensic medical assessment of fatal cases of acute methadone poisoning in patients admitted to a multidisciplinary hospital and establish the role of metabolic disorders in the occurrence of death.</p> <p><bold>MATERIALS AND METHODS</bold><italic>:</italic><italic> </italic>A single-center, retrospective, sample, cohort study was conducted. Medical records and data from forensic medical reports were examined. All included observations with a positive determination of methadone were confirmed in the framework of lifetime chemical and toxicological studies and during postmortem forensic chemical studies of the biological environment of corpses. For the final interpretation of the results and evaluation of their statistical significance, the Pearson’s Chi-square (χ<sup>2</sup>) criterion was used.</p> <p><bold>RESULTS</bold><italic>:</italic><italic> </italic>A comparative analysis of clinical and forensic data from most cases revealed consistent causes of death established in vivo and posthumously. Moreover, the leading pathological process that led to death was a combination of pulmonary and cerebral edema. The variability in the causes of death in some cases was due to the background pathology determined by thanatogenesis. The life expectancy of patients was determined by the effectiveness of intensive therapy (ventilator, infusion therapy, vasopressor, inotropic support, correction of metabolic disorders) and characterized by background pathologies and duration of hospital stay.</p> <p><bold>CONCLUSION</bold><italic>:</italic><italic> </italic>A number of main and related conclusions of practical forensic and clinical interest were drawn from obtained data. Notably, in all cases, the fatal outcome of methadone poisoning was associated with metabolic disorders and severe hypoxia. Moreover, the use of antidote therapy with naloxone and wait-and-see techniques without respiratory support in the treatment of such patients is unreasonable.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. Согласно современным научным данным, от отравлений метадоном в медицинских учреждениях умирает только 1,1% пациентов. В связи с этим имеющиеся на сегодняшний день судебно-медицинские представления об указанной нозологии получены преимущественно на основе секции трупов, умерших без предшествующего лечебно-диагностического процесса. Таким образом, в судебной медицине в значительной степени не изучены вопросы динамики и роли метаболических расстройств в танатогенезе острых отравлений метадоном, а также судебно-медицинской оценки таких случаев.</p> <p><bold>Цель исследования</bold> ― судебно-медицинская оценка смертельных случаев острых отравлений метадоном у пациентов, поступивших в многопрофильный стационар, с установлением роли метаболических расстройств в возникновении смерти.</p> <p><bold>Материалы</bold><bold> </bold><bold>и</bold><bold> </bold><bold>методы</bold>. Непосредственным материалом одноцентрового ретроспективного одновыборочного когортного исследования являлись медицинские карты и данные судебно-медицинских заключений. Все включённые наблюдения с положительным определением метадона были подтверждены анализом биосред трупов как в рамках прижизненных химико-токсикологических, так и в ходе посмертных судебно-химических исследований. Для конечной интерпретации результатов и оценки их статистической значимости был использован критерий Хи-квадрат (χ<sup>2</sup>) Пирсона.</p> <p><bold>Результаты</bold>. При сравнительном анализе клинических и судебно-медицинских данных в большинстве случаев выявлено соответствие причин смерти, установленных прижизненно и посмертно, при этом ведущим танатогенетическим механизмом был комбинированный патологический процесс ― отёк лёгких и отёк головного мозга. Случаи несоответствия причин смерти объяснялись преимущественно за счёт выхода фоновой патологии на первое место в процессе танатогенеза.</p> <p><bold>Заключение</bold>. Полученные данные позволили сделать ряд основных и сопутствующих выводов, представляющих практический судебно-медицинский и клинический интерес. В качестве наиболее значимых умозаключений можно отметить то, что во всех случаях смертельный исход отравлений метадоном был связан с глубиной метаболических расстройств при тяжёлой гипоксии. Продолжительность жизни больных определялась эффективностью интенсивной терапии (искусственная вентиляция лёгких; инфузионная терапия; вазопрессорная, инотропная поддержка; коррекция метаболических расстройств) и характеризовалась выходом фоновой патологии на первое место по мере длительности пребывания в стационаре.
