In nonobvious conditions and when it is crucial to determine the duration, such a polymorphic cellular infiltrate can be mistakenly considered as the result of a post-traumatic inflammatory reaction, affecting the assessment of the duration of the injury.</p> <p><bold>AIM</bold><italic>:</italic><italic> </italic>To study the composition of the cellular infiltrate in hemorrhages in rib fractures, taking into account their morphological features in cases of various causes of death at different periods after injury.</p> <p><bold>MATERIALS AND METHODS</bold><italic>:</italic><italic> </italic>A retrospective analysis of cases and an analysis of personal observations of deaths of individuals of different age and sex groups with different causes of death, but with anamnestic data of chest injuries, and of cases of isolated chest injury, aimed at forensic histological examination, were performed. The study sample was divided into main study groups according to established time intervals based on anamnestic data of brief circumstances of the case indicated in the referral documents (“those who died at the scene,” “chest injuries as a result of resuscitation measures,” “victims who lived up to 6 hours,” “victims who lived up to 12 hours,” and “victims who lived up to 24 hours”). The forensic (histological) examinations of deceased individuals with fractured pelvic bones and fractured skull bones and examinations of those with hemorrhages in soft tissues with stab wounds of the chest were compared. Additionally, the comparison sample included deaths of people of both sexes, and different age groups and with various causes of death in different periods after injury. From this sample, subgroups similar to those in the main study were formed for comparison.</p> <p><bold>RESULTS</bold><italic>:</italic><italic> </italic>Analysis showed that in most victims who died at the scene of the incident, immature cellular forms were found in hemorrhages in the soft tissues of the intercostal space, which gradually decreased over time. Morphological study and analysis of hemorrhages into soft tissues showed no immature cellular forms in traumatic hemorrhages of other locations in all comparison groups.</p> <p><bold>CONCLUSION</bold><italic>:</italic><italic> </italic>Notably, immature cellular forms in the polymorphic cellular infiltrate and hemorrhages in soft tissues from the intercostal space are not a reactive post-traumatic process and cannot be used to estimate the time and duration (survivability) of the injury. Moreover, the preservation of immature cellular forms in the hemorrhage area has been found to change over time with their gradual decrease by 12 hours.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. Практические наблюдения указывают на особенность кровоизлияний при переломах рёбер: нередко в исследуемом клеточном инфильтрате обнаруживаются незрелые клеточные формы. В условиях неочевидности и при необходимости определения давности повреждения такой полиморфно-клеточный инфильтрат может быть ошибочно воспринят как результат посттравматической воспалительной реакции и влиять на оценку давности повреждения.</p> <p><bold>Цель исследования</bold> ― изучить состав клеточного инфильтрата в зоне кровоизлияния в области переломов рёбер с учётом их морфологических особенностей в случаях различных причин смерти у пострадавших в различные периоды после получения травмы.</p> <p><bold>Материалы</bold><bold> </bold><bold>и</bold><bold> </bold><bold>методы</bold>. Проведён ретроспективный анализ случаев и анализ собственных наблюдений летальных исходов у лиц разных возрастных и гендерных групп с разными причинами смерти, но имеющих анамнестические данные о повреждениях грудной клетки, в том числе изолированную травму грудной клетки, направленных на судебно-гистологическое исследование. Исследуемая выборка была разделена на основные группы исследования по условно установленным временным промежуткам, опирающимся на анамнестические данные из кратких обстоятельств дела, указанных в направительных документах («умершие на месте происшествия»; «повреждения грудной клетки в результате проведения реанимационных мероприятий»; «пострадавшие, прожившие до 6 часов»; «пострадавшие, прожившие до 12 часов»; «пострадавшие, прожившие до 24 часов»). Для сравнения подобраны случаи судебно-медицинской (гистологической) экспертизы при травмах с переломом костей таза, переломом костей черепа, кровоизлияниями в мягких тканях при колото-резаных повреждениях грудной клетки. В выборку для сравнения вошли также случаи смертельных исходов лиц обоего пола, разных возрастных групп, различных причин смерти в разные периоды после получения травмы, из которых сформированы подгруппы, аналогичные основному исследованию.</p> <p><bold>Результаты</bold>. Установлено, что у умерших на месте происшествия в наибольшем числе случаев в кровоизлияниях в мягких тканях межреберья обнаруживаются незрелые клеточные формы с постепенной их убылью с течением времени. Морфологическое изучение и анализ кровоизлияний в мягкие ткани во всех группах сравнения показали отсутствие незрелых клеточных форм в травматических кровоизлияниях другой локализации.</p> <p><bold>Заключение</bold>. Незрелые клеточные формы в полиморфно-клеточном инфильтрате, в кровоизлияниях в мягких тканях из области межреберья не являются реактивным посттравматическим процессом и, соответственно, не могут использоваться для оценки давности травмы. Доказано, что сохранение незрелых клеточных форм в зоне кровоизлияния в течение времени изменяется с их постепенной убылью к 12 часам.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold>论证</bold>。实际观察表明，肋骨骨折出血有一个特点：在检查的细胞浸润物中经常发现未成熟的细胞形态。在不明显的情况下，当需要确定受伤时间时，这种多形性细胞浸润物可能会被误认为是创伤后炎症反应的结果，从而影响对受伤时间的评估。</p> <p><bold>该研究的目的</bold>是调查肋骨骨折出血区域细胞浸润物的组成，同时考虑到不同死因的受害者在受伤后不同时期的形态特征。</p> <p><bold>材料与方法</bold>。我们对案例进行了回顾性分析，并对自己观察到的不同年龄和性别组别、不同死亡原因、但有胸部损伤（包括孤立性胸部创伤）病史数据的人员的死亡结果进行了分析，并将其转交给法医组织学检查。研究样本在转介文件中指定的病例摘要中的病史数据的基础上按照条件时间间隔划分为主要研究组（“当场死亡者”；“因抢救措施导致的胸部受伤”；“存活时间不超过 6 小时的受害者”；“存活时间不超过 12 小时的受害者”；“存活时间不超过 24 小时的受害者”）。为了进行比较，我们选择了对骨盆骨折、颅骨骨折、胸部刺伤软组织出血的法医（组织学）鉴定案例。比较样本还包括男女、不同年龄组、不同死因的人在创伤后不同时期的死亡结果案例，并从中形成与主要研究类似的分组。</p> <p><bold>结果</bold>。研究发现了，在大部分事故现场死亡者的肋间隙软组织出血案例中，可以发现未成熟的细胞形态，而且随着时间的推移会逐渐消失。对所有对比组软组织出血的形态学研究和分析表明了，其他部位的创伤性出血中不存在未成熟细胞形态。</p> <p><bold>结论</bold>。多形性细胞浸润物和肋间软组织出血中的未成熟细胞形态不是创伤后的反应性过程，因此不能用于评估受伤时间。事实证明，出血区域中未成熟细胞形态的保存会随着时间的推移而发生变化，12 小时后会逐渐消失。</p></trans-abstract><kwd-group xml:lang="en"><kwd>chest trauma</kwd><kwd>hemorrhages</kwd><kwd>bone marrow cells</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>травма грудной клетки</kwd><kwd>кровоизлияния</kwd><kwd>клетки костного мозга</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>胸部创伤</kwd><kwd>出血</kwd><kwd>骨髓细胞</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Khadjibaev AM, Ismailov DA, Shukurov BI, Isakov ShSh.
